What I need:
The personal information I collect is name, address, date of birth, email address and telephone number. I also require developmental, medical and school information and keep records of any contact I may have with you. This ensures I can provide you with an excellent service.
Why I need it:
I need to know your basic personal data for administration purposes should you commence service for an assessment, treatment or training work and to keep you updated with relevant information that may benefit you.
How I collect it:
Data is collected from you when you contact Sensory Therapy via an enquiry on the web form, from a phone conversation and third party referrals.
What I do with it:
All personal data is processed by me. No third parties have access to your personal data unless you give me express and written permission (this will be other professionals involved in your child’s care). I will not use your data for any other purposes such as mail shots etc.
How long do I keep it?
Sensory Therapy only keep your personal details for as long as necessary. Once the information is no longer required or you do not wish to hear from me, it will be disposed of securely. This is always discussed with clients from the onset of dealing with Sensory Therapy.
What are your rights?
If at any point you believe the information I process on you is incorrect or would like to update your contact preferences, you can request to see this information and have it corrected or deleted by contacting me. If you wish to raise a complaint on how I have handled your personal data, please contact me and I will investigate the matter further. However, Sensory Therapy will always treat sensitive data with respect and in accordance with the Data Protection Act.
If you are not satisfied with my response or believe I am not processing your personal data in accordance with the law you can complain to the Information Commissioner’s Office (ICO).
